Bag of worms, Varicocoele

The bag of worms inside a varicocoele@
One of the candidates who went for the final MBBS clinicals short case, was given a Varicocoele to be examined and diagnosed. He did the correct thing making the patient stand up and feeling the scrotum. He made the patient to lie down and examined the patient's abdoman for any intra-abdominal lumps. The examiner was pleased and said
" OK, what is your diagnosis?"
The answer came "Varicocoele Sir."
Then the examiner asked "Why do you say that?"
The reply was "I felt a bag of worms Sir".
"The puzzled examiner asked "What do you mean by a bag of worms?"
The reply flummoxed the examiner " It is full of worms Sir"
The examiner asked in surprise "What kind of worms?"
The answer was "Filarial worms Sir".
You decide whether this candidate passed the Finals Surgery at the MBBS.

For the uninitiated
A varicocoele is a blood filled venous tortuosity in the scrotum. It contains blood but feels 'like a bag of worms'.
